Don’t rush to your first appointment. You can take your time and fill out your new patient forms at your convenience in your home. This will also ensure we have all the information we need.

There’s more than one way to pay for your care. With dental insurance coverage, we will file your claim for all your benefits. For your payment, we accept major credit cards and offer payment plans that are interest-free. You can apply in our office for CareCredit, Lending Club, and iCare services that do not require a credit check.

If you do not have insurance benefits, we have partnered with Quality Dental Plan to provide a customized and comprehensive membership plan with 100% coverage for your preventive care, such as cleanings, exams, and X-rays.
